ADULT BASED SERVICES
Adult Day
Transitions Behavioral Health offers Adult Day Services for individuals with developmental disabilities.
The program is designed for individuals who have completed high school and are looking for a collaborative social environment to participate in. Our day program is focused on continued educational interests as well as hands-on social and recreational activities. Through personalized plans, we work to build confidence, explore new activities and increase independent living skills. Transitions Behavioral Health offers opportunities for growth through the addition of behavioral supports, communication supports and music therapy services.
Medicaid Outpatient Services
Transitions Behavioral Health offers an array of Medicaid funded Outpatient Services to meet the needs of children, adolescents and adults with Behavioral Health concerns.
Assessment and Referral
Includes prescreening, psychosocial assessment, determination of need, referral to appropriate level of care and development of an ITP(Individualized Treatment Plan).
Behavior Support
Therapeutic Behavioral Services are available to provide consultation, interventions using evidence-based techniques, identification of strategies and treatment option, restoration of social skills and self management of daily functioning.

Case Management
Counseling- Can be provided to address a variety of needs, including, situation stressors, family relations, interpersonal relationships, mental health issues, life span issues and social skills development.
